Application Materials
We have a rolling application process, so it is not too late to get your application materials
in to Eastside Prep. An application includes:
Family Forms
- Basic Application
- Application fee ($50.00 - checks payable to Eastside Preparatory School)
- Parent Questionnaire
- Student Questionnaire
- Personal Reference
School Records
- Grades/transcript from current school year
- Grades/transcript from previous two years
- Standardized test results (if available)
- Discipline record (if applicable)
- Two Teacher's Evaluation Forms
Independent School Entrance Exam (ISEE) Scores:
- Register for the ISEE at www.iseetest.org. Eastside Preparatory School’s code is 481714.
Student Visit Day
All applicants to Eastside Prep will spend a day visiting classes. These half-day or full-day visits typically take place in January and February, but can continue through the end of the school year. Applicants will be contacted to schedule a visit after the application form and parent and student questionnaires have been received by the admissions office.
Parent Interview
All parents of applicants will meet with a member of the Admissions Committee during the application process. Parents will be contacted to schedule an interview as soon as all pieces of the application have been received.
